How much a trademark license agreement costs depends on the complexity of the arrangement and whether it’s prepared from scratch by an attorney or from a template.

If it’s a straightforward exclusive/non-exclusive license that covers a single territory and class, it should cost around several hundred to a few thousand USD, depending on the jurisdiction and the firm involved. If it’s a more complex arrangement that covers multiple territories, sublicensing rights, royalty structures, and quality control provisions, it will probably cost a lot more.

Using a generic template carries legal risk; a license agreement that does not correctly reflect the arrangement or that lacks necessary provisions (such as quality control clauses, which are required in some jurisdictions to maintain the trademark's validity) may undermine the protection it is intended to provide. Having an attorney at minimum review any template before signing is advisable.
