Can a trademark specimen get rejected?

Yes, your specimen can be rejected if it doesn't meet all of USPTO's requirements. You can find the list in the question What requirements does a trademark specimen have to meet?.

However, if that happens, you can submit a substitute specimen. The USPTO examiner will describe the reasons for rejecting your specimen in the office action. You can use this information to prepare a new specimen.
